EMERGENCY TRAVEL DOCUMENTS (ETC)

For the acquisition of Emergency Travel Certificate (ETC), please visit the Consular Section, Nigeria High Commission at No. 1 Musa Yar’adua Crescent, Hill Cot Junction, Freetown with the following requirements;

• FOR MINORS

  •  Two Passport Photograph
  •  Original copy of Child’s Birth Certificate
  •  Copy of travel plan/itinerary
  •  Copy of Parents Nigerian Passport data page
  •  Letter of Consent signed by/from both Parents addressed to the Nigeria High Commissioner, Freetown, Sierra Leone. Signatures should match the signatures on their respective passports and names should match those on the child’s birth certificate
  •  Money Order to be made available at arrival to the High Commission
  
  
  • Note: In case where only the mother appears on the birth certificate, she will have to present an affidavit from the court that she has full custody of the child/children

• FOR ADULTS

  •  Application letter addressed to the Nigeria High Commissioner, Freetown, Sierra Leone
  •  Two Passport Photographs
  •  Copy of Expired or Lost/Stolen Passport
  •  Police Report for Lost/Stolen Passport
  •  Copy of travel plan/ itinerary
  •  Money Order to be made available on arrival to the High Commission
  
  
  • Note: In case where only the mother appears on the birth certificate, she will have to present an affidavit from the court that she has full custody of the child/children

BURIAL TRANSIT PERMIT

• Requirements
  • - Four certified copies of death certificate (if the certified copies do not show a cause of death, the Consulate will require a photo of the original showing the cause or a letter of non-contagious w/cause from attending physician)
  • - Burial permit/w raised seal
  • - Letter of embalming on funeral home letterhead, letter must state that the remains were placed in a metal sealer casket
  • - Letter of non-contagious disease from State or County Health Official stating that the deceased did not die from a communicable disease and that there were no contagious diseases present in the area at the time of death, signed and notarized
  • - Separate statement from the Funeral home that the casket contains only the remains of the deceased, A signed and notarized
  • - Passport of deceased
  • - Metal sealer casket
  • - Outer wood box required/no-inner liner
  • - No Consul inspection is required. The Consulate will issue a letter of permission to ship
  • - A fee (money order) A made out to the Consulate General of Nigeria. Processing time 3 business days from the day of receipt in the Immigration Dept
  • - Letter from the family verifying that they will pick up the body upon arrival in Nigeria with the name, address and phone# of the consignee.
  • - Prepaid self-addressed envelope has to be UPS mail.
